The plugin appears in the filter menu (not the effects menu, it is not a live effect plugin). The plugin displays a dialog that offers presets as well as the two point / one point dropdown
The plugin can be used to create normal paths (lines) but it can be used in guideline mode. Once in guide lines, the guidelines can be used with created paths and converted text to create paths in the correct perspective, for dramatic zooms and more.
The grid can also be turned into a brushed grid simply by selecting the path and selecting a brush type (you might have to change the stroke weight to get decent results), likewise with styles.
The grid created can be further manipulated by other plugins such as the free transform tool or the free distort plugin (though this might create lines that breakaway). The lines can also be modified by the envelope warp distortion to create arced grids or fish eye grids. The grid can also be modified by third party plugins such as the ones from graphicxtras (creating circular effects or warped illustrations)
The grids can also be combined to create a multiple extended grid. It is best to create a basic grid and then copy and paste to create new grids (the more complex the grid, the harder it will be to align the grid - if required)
You will see a small VP crossbeam. You can use this as the center of scaling / rotating paths for perspective. The VP is a guideline so you can't edit it but you can clear guidelines view menu
The plugin is split over five different dialog pages: The first page relates to the setup of the grid size / dimensions and the location of the vanishing point. At the side of the dialog, two dropdowns. The first sets the one point or two point mode for the perspective grid. The second dropdown is the presets (an excellent way of trying out the plugin)
The second page is for setting up the structure such as the project lines, the number of frames, convergence, and others
The third page is for setting the transformation such as rotate.
The other pages relate to the settings for stroke weight / fill color / swatches / and much more
